Neck pain

Physical therapy for neck pain in Toronto

Neck pain rarely arrives alone. It brings stiffness turning one way, an ache between the shoulder blades, and often a headache that starts at the base of the skull. Treating the neck in isolation is why so many people get temporary relief and no change.

  • Upper back assessed, not just the neck
  • Headache link tested directly
  • No referral needed in most cases

What is going on

The neck is usually paying for the ribs

A neck that cannot rotate is often a neck doing work the upper back has stopped doing. The thoracic spine loses rotation and extension quietly, nothing hurts there, and the segments above take up the slack until they object. That is why mobilising the mid back frequently changes neck symptoms in a single session.

The second common driver is endurance rather than strength. The deep neck flexors fatigue, the bigger muscles take over, and by four in the afternoon everything aches. See desk work and posture and manual therapy.

Posture itself matters less than people are told. Positions are only a problem when you hold one of them for hours without variation, which is why the fix is usually a timer rather than a new chair. More in pain versus soreness and therapeutic exercise.

The plan

How Toronto physical therapy treats neck pain

  1. Test the neck and the ribs

Rotation, side bending and thoracic extension. The limitation is often a level or two below where it hurts.

  2. Restore rotation first

Hands-on mobilisation to the mid back and neck, which usually buys range within the first visit or two.

  3. Build endurance, not strength

Deep neck flexor and scapular work in small, frequent doses. Heavy loading is rarely the answer here.

  4. Change the day, not the posture

Variation beats perfect positioning. We look at how long you hold things rather than how you hold them.

Related headaches

When the headache is coming from the neck

A headache that starts at the base of the skull, sits on one side, and can be reproduced by pressing the upper cervical joints is usually cervicogenic, which means the neck is generating it. Those respond well to treating the neck, and it is worth testing properly rather than assuming.

Migraine is a different problem with a different mechanism, though a stiff neck can be a trigger for it. Mixing the two up wastes weeks, so the assessment separates them before any plan is written. See headaches and jaw pain.

If headaches are frequent, severe, or have changed in character recently, that is a medical question before it is a therapy one, and we will say so rather than starting a plan. More in working with your doctor and when to see a therapist.

Common questions

Toronto neck pain physical therapy questions, answered

What people ask when the neck has been stiff or sore for weeks.

Less than you have been told. Sustained positions matter more than the shape of them, and people with excellent posture get neck pain too. Changing how often you move usually helps more than changing how you sit.
Yes, and it is worth testing rather than assuming. A cervicogenic headache typically starts at the base of the skull, stays on one side, and can be reproduced by pressing specific joints during the assessment.
Possibly, though it is rarely the main factor. If you wake worse than you went to bed, pillow height is worth trying. If the pain builds through the day instead, the pillow is not your problem.
Generally not dangerous, but it tends to be unsatisfying over time because it moves the segments that already move. The stiff ones stay stiff, which is why the urge returns within the hour.
Often faster than lower back pain. Four to six weeks is typical for a straightforward case, and many people notice a change in rotation during the first or second visit once the mid back is addressed.
Pain after significant trauma, pins and needles in both arms, clumsiness in the hands, unexplained weight loss or fever all warrant a physician first. Those are referral questions, not therapy questions.

Neck pain from a desk job. They assessed my setup, gave me three exercises and worked on the stiff segments. Explained each step.

They tested which movements reproduced the arm tingling and tracked how far I could turn my head each review. It got measurably better.

No cracking, no promises. Just hands-on work for the stiffness and a loading programme for the weakness. It took six weeks as they said.

The therapist told me plainly that the posture advice I had been given before was not the issue. The strength work was.
